·         If you have never applied to SUSD #48 before select the button labeled "Create New Account and Apply."  You will be prompted to create an applicant profile before you can start the application.

·         If you have applied in the past to SUSD #48 enter your username and password and the system will retrieve your previously submitted applications and data.

Requirements:

Volunteers must be at least 18 years of age.  

All volunteers will be required to complete the Scottsdale Unified School District #48 COVID-19 Waiver, Release, and Assumption of Risk Form.

Volunteers will be required to wear a mask while on campus – regardless of vaccine status.

Volunteers will be required to submit to a Background Check. Please note that this process typically takes between 24 – 72 business hours to complete.

• If you do not have a social security number (SSN) please enter all zeros (000-00-0000) under the Background Screening portion of the application.

IMPORTANT:

**Regardless of your vaccination status, if you begin to show ANY COVID-like symptoms, we ask that you do not go to any school campus**

 If you start with ANY symptoms after just completing your negative COVID test, do not go to a school campus. Contact the school nurse for further direction. COVID-like illness would include ANY of these symptoms: • Fever • chills • body/muscle aches • GI complaints • Nausea • Vomiting • Diarrhea • Cough • Headache • breathing difficulties • new loss of taste or smell • sore throat • runny nose or congestion.
